Outreach & Marketing:

Manages all aspects of the Community Counseling & Development Center (CCCD), follows the agency mission, sets goals and objectives for the CCCD.

Participates in the development of long-range strategic planning.

Represents the CCCD and Central Michigan University in the community and peripheral environments and develops and supports ongoing partnerships.

Develops, plans, and implements strategies to manage clinic growth and support progress toward self-funding and sustainability of the program.

Reviews and recommends changes in program services to meet client needs, with particular emphasis on services for individuals with lack of access to mental health support.

Develops and implements an ongoing marketing strategy for client referrals, ensuring client referrals meet CACREP accreditation requirements for 40 hours of face-to-face counseling per counselor-in-training.

Administrative Duties

Oversees and ensures the proper handling and management of mental health records for clinical reporting and research purposes.

Collaborates with other center staff who provide clinical supervision to counselors-in-training to ensure all progress notes, treatment plans, and discharge summaries are reviewed and signed.

Works with the HIPAA Compliance Office, IT Health Care, and the CMU Counseling Program to ensure HIPAA compliance.

Attends Counseling Program meetings and provides ongoing verbal and written reports to faculty regarding any student concerns.

The Center for Community Counseling & Development (CCCD) is a practicum training lab for addiction, clinical mental health, and school counselors-in-training enrolled in the Central Michigan University Counseling Program. The CCCD serves two missions; (1) to provide a state-of-the-art training facility that allows students to develop and enhance counseling skills and (2) to provide no cost counseling services via face-to-face or tele-mental health services to the Central Michigan University community, Mt. Pleasant and surrounding areas, and across Michigan.

The Police Officer position is a member of the Police Officers Association of Michigan (POAM) union. Maximum pay is available after one or two years of employment, depending on the rate of pay received upon hire. Wage rate information is available in the POAM union contract. In addition, a $1.00 per hour shift premium is applied for night hours worked between 7:00p.m. to 7:00a.m. We also compensate Officers when working in a higher classification such as filling in as a Sergeant, or when working as a Field Training Officer or on Special Assignment. Once our new Officers have completed their 16-week Field Training Program they will be placed on a standard 12-hour shift. The current day shift works from 7:00 a.m. to 7:00 p.m. and the night shift works from 7:00 p.m. to 7:00 a.m. We have a total of four shifts, a day and night shift for Sunday, Monday, Tuesday, and a day and night shift for Wednesday, Thursday, Friday. Each day and night shift alternate working on Saturday. On an annual basis our Officers work a standard 2,184 hours with a variety of overtime opportunities.

Mt. Pleasant's Police Department patrols approximately 8 square miles with 28 sworn officers. Position Summary:
The Family Nurse Practitioner (FNP) or Physician Assistant (PA) is responsible for direct patient care and for other administrative duties as designated by the Dean and his/her designee. Required Qualifications:
Graduate of an accredited Nurse Practitioner or Physician Assistant educational program.
One year of clinical experience.
American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) certified for FNPs.
National Commission on Certification of Physician Assistants (NCCPA) certified for PAs.
Existing unrestricted license to practice in Michigan, or able to be licensed in Michigan.
Current Advanced Trauma Life Support (ATLS) and Advanced Cardiovascular Life Support (ACLS) certification or planned attainment.
Excellent practice history; ability to be insured for liability purposes.

Preferred Qualifications:
Two years of post graduate clinical experience.
Experience working with Electronic Medical Records and computers.
Experience in an Academic Medical Center, Urgent Care, Walk In Clinic or teaching environment.

Duties & Responsibilities:
Practices medicine using sound medical judgment and adheres to all ethical considerations of the practice of medicine; knows limitations and seeks consult when advisable; keeps abreast of current technologies and industry standards.
Follows all internal policies and procedures; is accountable for completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing while being solely responsible for all proper documentation and assigning of diagnosis codes suitable for ICD-10.
Advises administration of any issues that may impact the practice.
Sees patients in a timely and efficient manner per current University Health Services (UHS) protocols, while adhering to acceptable guidelines for time frames for patient visits, stays reasonably on time for scheduled appointments in family practice throughout the day, unless exceptional circumstances occur; patient relations, consumer satisfaction and long standing organization goals of staying on time necessitate this important consideration.
Charting in the patients medical record is to be completed on the same day as the patients visit as well as documentation suitable for proper ICD-10 coding.
Responsible for providing episodic and chronic care, as well as screening and educational assistance to the clinics patients.
Recognizes diversity of patient population and treats all warmly and kindly.
Works in harmony with other members of the healthcare team to provide quality service to patients.
Provides quality care to patients by following the most current evidence-based guidelines and procedures of practice.
Following the clinics evidence-based guidelines, performs common vaccinations and diagnostic screenings.
Takes part in quality assurance audits, including continuing education, CP review, medication administration review and peer review.
Ensures UHSs objectives regarding financials, patient engagement and healthcare quality are followed.
Promotes University Health Services to patients and customers.

SUMMARY

General Description: Work collaboratively with other treatment professionals within a clinical team to engage, educate, communicate, and coordinate care with consumers and families specific to their mental and physical health needs for a whole health approach to care. Responsible for coordinating an array of services under the functions of case management/supports coordination including person centered planning, assessing, linking and coordinating of services, monitoring, and advocating responsibilities regarding the needs of individuals. Case Managers work with individuals and families with intellectual/developmental disabilities, acute or long-term mental illness, and co-occurring substance use disorders. Their aim is to instill hope and provide care and support using a person-centered approach to help individuals and families establish and achieve personal meaningful goals.

Responsibilities and Duties: Uses guided discovery to help individuals/families identify the presenting problem, understand their needs, and build rapport. Completes psychosocial assessment to identify services and supports needs. Supports individuals/families in the development and review of a Person-Centered Plan which includes interventions to enhance or restore the capacity for social functioning, linking to needed services and resources, and monitoring efficacy of the plan and service delivery. Demonstrates understanding of the Home and Community Based Settings Final Rule within the person-centered planning process. Engages, activates and motivates individuals and families to foster growth and opportunities for change to assist individuals and families to achieve their desired outcomes. Provides support, skills training, and psychoeducation specific to identified areas of focus. Actively works with the treatment team to engage and build rapport between the consumer and all members of the treatment team for increased access to care and services, decreased wait times, and improved treatment outcomes. Case Managers are responsible for monitoring the consumer's overall status and communicating updates with other members of the treatment team. Conducts crisis screenings and interventions, as well as crisis and safety planning. Provides backup to Crisis Mobilization Intervention Team (CMIT) as needed. Assists individuals/families in finding necessary resources related to social determinants of health, such as housing, food security, education, and transportation. Advocates on behalf of the person with other community partners. Addresses health and safety issues using a whole-health approach with the individual (and, if appropriate, the guardian) by coordinating care with other healthcare providers and insurers. Assists the individual in becoming an active member of their community. This will be done by linking the individual with needed services, community activities and coordination of same. Monitors the person-centered process through contacts with the consumer, consumer's family, community members, and other evidence. Completes the necessary record keeping functions as defined by CMHCM and assists the individual in accessing all potential entitlements needed. Provides back-up to other staff, participates in staff meetings, attends court hearings, coordinates with other agencies and provides consultation to staff. May also include tasks specific to serving consumers outside of the agency's six county catchment area. Examples of these duties include: Monitoring state hospital admissions/continuing admission criteria, completion of consult notes related to Center for Forensic Psychiatry (CFP) referrals, and monitoring of Not Guilty for Reason of Insanity (NGRI) cases.

EMPLOYMENT STANDARDS

General Education:
College: Bachelor's Degree

Job-Related Experience: Sufficient experience to perform the position at a satisfactory level required. Experience related to the appropriate recipient population preferred. Three years of experience with children and families preferred when providing services to children.

Licenses/Certification: LLBSW or LBSW preferred but not required. Valid driver's license while operating vehicle on agency business.
